Changes in insurance information make it hard for anyone to keep up with and we understand that. That is why knowing your specific plan’s coverage and understanding your co-pay and deductibles will help you determine your out-of-pocket expenses. Patients without Insurance Coverage

Payment at the time of service is required. We offer a 10% discount to patients paying for their services in full during their visit. Short-term payment plans are available but must be requested before the services are performed.

Which Insurance Companies Do You Bill?

We accept and bill most major insurance plans as a regular service to our patients. All we ask is that you present your insurance card at each visit so that we can verify the correct billing info for your visit. You are responsible for providing us with the correct billing information. See our list of insurance companies that we contract with.

What Do “Contracted” and “Allowables” Mean?
Contracted insurance means that we have agreed to the particular insurance company’s fee schedule for payment of services. Allowable is just another term used for this. Although we accept the insurance allowable, there is usually a portion, percentage, deductible, or co-pay that the patient is still responsible for.
Who Is Responsible for Payment?

If you are 18 or over, you are legally responsible for your own account, regardless of who you live with, who claims you as a tax deduction, or who has the contract with the insurance company. If the patient is under 18, BOTH parents, despite divorce or separation agreements, OR the legal guardian are responsible for payment.

Although we are happy to assist you as best we can, we ask that you be fully responsible for knowing the specifics of your particular insurance coverage. Examples of these specifics include: Co-pays, deductibles, second opinions, pre-authorizations, preferred providers, referrals, covered and non-covered services, and preferred hospitals and facilities.

What If I Need to See a Specialist?
You and your primary care physician together will determine the need to see a consulting specialist. It would be very helpful for you to look over your policy and become familiar with your insurance plan and its requirements for a referral to a specialist.
When Is Payment Expected from the Patient?
We do expect payment at the time of service. If we are on contract with an insurance company, we expect the patient’s portion of our fee, co-pay, or deductible at the time of service. We accept cash, check, Visa, Discover, and MasterCard as payment options.
